"Help!  Is there a Doctor in the house?"

Dear doc,
Is it possible for joints (elbow in paticular) to just pop in and out of place.  My 9 year old dd has this happen frequently, is this common and should we worry? thank you.

Hmmm. This concerns me. There are several conditions in the pediatric elbow that have this symptom, one is Nursemaids elbow which occurs in kids 5 years or less and a second is an elbow dislocation which occurs in the 11-15 year age range. A third is osteochondritis dissecans and a fourth may simply be a loose body or just loose joints!!

Nursemaid's elbow is a common condition in young children .The injury occurs when a child is pulled up too hard by the hand or wrist. It is often seen after someone lifts a child up by one arm up. (For example, when trying to lift the child over a curb or high step.) Swinging a young child from the arms while playing can also cause this injury.

Most kids are upset when it happens but do not have severe pain. They often show a lack of use of the arm. The elbow (radial head) is subluxed partially out of joint, but a simple movement in the office can reduce it. Most kids do not have repeat events as long as caregivers are aware of the mechanism. The condition ceases to occur after 5-6 years of age due to a maturity of the elbow.

An elbow dislocation is much more traumatic generally occurring after a fall on the outstretched hand in the 11-15 year age group, although it may occur in adults as well. The entire elbow "pops" out and needs to be reduced generally with an anesthetic! Unfortuantely because of ligament injury, recurrent elbow dislocation, though rare, may still occur. An unstable elbow may make a "popping" news during normal activities.

The third condition that may give a sense of "popping out " is osteochondritis dissecan(OCD), a cartilage condition sometimes referred to as Panners syndrome or Little Leaguer elbow. In this condition the cartilage in the elbow may break free and cause the joint to "pop" or feel like it may "pop out"! This condition tends to occur in the 7-12 year pld throwing athlete. So, needless to say if your DK is not throwing regularly, this condition is even more unlikely than the other two.

Finally, a loose piece of bone or cartilage may cause similar symptoms. A loose body is however often related to OCD! Loose joints in and of themselves may allow structures to grind and cause a similar phenomenon.

The bottom line is if the elbow feels like it is "popping out" and especially if there is pain, get it checked by an Orthopedist. An Xray or MRI may be indicated and diagnostic of the condition. There is a good chance that this condition is of no significance, but when in doubt, check it out!!

Good luck!!

WPS FAMILY CIRCLE FOUNDATION!!!

No news this week, which is great, but keep those from past Pulses' in your thoughts and prayers.

"COACHS CORNER!"
Why are some coachs so arrogant and never blame them selfs for anything?
Very good question. As any of us know its much harder to be humble and talk fault in yourself then it is to blame others. thats the simple answer to this. Another small reason is the players take on the personality of the coach so if he/she is constantly blaming themselves that might translate to the players losing faith in the coach, or blaming themselves.

However, a good friend of mine use to always tell me "if my team wins its because of the players, if they lose its because of me". The reason why he said this is because he had one of the best teams in the state. So if he had one of the best teams with players that could do almost anything then if they lost instead of blaming them it meant he hadn't prepared theem properly. However, If he would have gone to the players and told them this they may start to lose faith that he can really lead them all the way to the top.

Basically I personally think coaches blame themselves more than you know, they just don't want to admit it to other because they don't want parents and players to lose faith in their abilities. Its a tough spot to be in.

"VIEW FROM BEHIND THE WHISTLE"

"Questions for the Ref"
Is reffing something someone can do for a living as an adult?  I am 17 and getting ready to graduate next year. Could I make a living from reffing and skip college?
Short answer: NO you can't skip college – period.

Longer answer: Very, very, very few can referee as a full-time profession.  If you are among the best of the very best, it is conceivable to find such a thing as a full-time gig in a European league.  The MLS hired 4 full-time referees last year, but I couldn’t support my family on their wages.

High level professional working referees, as opposed to USSF employed referee administrators (old guys back to refereeing U10 games), work mostly as independent contractors for individual game match fees and travel/expense stipends.  Since one typically can't work more than one quality professional match per week, it's not possible to earn a living as a referee unless directly employed by a league.

These high level referees do not work the games for the money (same as most of us dirt level referees).  Brian Hall, one of our best USSF/FIFA referees (now in forced FIFA retirement after committing the sin of turning 45), lists as his motivation his passion for the game along with the challenges and opportunities that came to him through refereeing.  This includes meeting the best players in the world; watching major World Cup action from 3 to 10 yards away while on the pitch; traveling and making friends from all over the world.

Refereeing can be many things: a way to gain new skills; be involved in higher level games than we could play; help the community and the sport; travel with expense offset; make enduring friendships.  It is NOT a way to make a living.  But it is an EXCELLENT way to earn spending money while you're in college!!

OTHER NEWS AND GOSSIP AND TIDBITS:

"RUMOR MILL"
* Heard from a Sun City Parent that Gary was extremely rude after the game to the Sun City Coach. Made excuses for the loss and a few other comments that were out of line. He just couldnt let the other team have their win and enjoy it.

*A DOC might be fired?  :o

* Heard that a new rule for soccer will be enforced this year.
Any parent or Coach that shows unsportsmanlike conduct directly before, during and directly after their match, will forfeit that game and those involved will have to do 5000 hours of community service for soccer.

NEWS FROM SPOKANE!
New Soccer Field Construction Begins Soon - Spokane, WA

Construction of new soccer fields and a softball complex at Dwight Merkel will begin in November 2008 or early Spring 2009.  Andrew Rypien Field, the South Complex, and other parks and school fields will be used during the construction, which is expected to take up to two years.  Collaborative efforts are underway with Spokane Parks and Recreation and SYSA to secure other fields for SYSA games and practices during construction.

"Dear Canuk"
Hey skunk bear. Do you read much about American politics? If so, what is your opinon?

It is just pathetic that you ameriCAN'T rubes look to this guy for advice on picking your sorry ass country's next leader. But since this is the only person that had enough CANadian horse bells to ask the question, this guy will answer.

BaLack Obama-This guy is an above average speaker but is too different for all the low-mentality hillbillies you have running around in spoKan't alone. The love train would almost bet one his favorite squirrel covers that this rube will not get picked by all the white supremacist nazis that still enjoy quarters in your sorry country and he will wind up broken and begging at carnivals for elephant ears and chicken wings.

John McOldasdirt-This guys old as John Macdonald, think old johnny even cruised with Macdonald in Nova Scotia scoping out bargoyle tarts. What this guy loves about him is how he treats woman like REAL men should! This guy will buy him a pint or two if he lives long enough. His jr prez pick is good looking enough that the love train might consider shaving her dish rack, since johnny can't even make the blue pill work anymore.

In this guys opinion, both losers need to bow down to the PM and accept they will never have a country as good as CANada!

If this guy had to pick a choice he would pick mcgeezer. At least he is going to have some hot ass in the wackhouse, rather it is the old tart or the young one bubs!
>:(

GOOD OF THE GAME -or- GOOD OF THE NAME

Topic: Tyncastle, a USSF affiliate,  was denied affiliation with WSYSA by the local association.  This means that WSYSA affiliated teams and Tyncastle may only play each other in unrestricted USSF sanctioned tournaments and Tyncastle may not play in any WSYSA sanctioned league or have practice games with WSYSA teams.

You decide ... was this decision for the Good of the Game or the Good of the Name?

"THINGS THAT MAKE YOU GO MMMMMMMMMMMMMM"?
Ten Game Fall Season Set For Washington Youth Soccer’s State League (Tuesday, August 19)

After evaluating the current league structure, Washington Youth Soccer felt three
components needed to be addressed in order to best meet the needs of the players and
program.

First, Washington Youth Soccer incorporated the research and
recommendations given by USSF which proved that players had a higher rate of
development when they practiced more and had fewer games. Thus it was decided that a
shorter ten game season with a re-designed format would strengthen development and
provide opportunities for better competition.

A shorter ten game season results in fewer games and more practices?
  • Western Washington - Fewer games yes but more practices?  Teams no longer need to prepare for a January State Cup.  With the season ending by Thanksgiving, how many teams will practice in December and if so what for?
  • Eastern Washington - Fewer games well ok.  But for most teams the season has just got longer by extending it through Thanksgiving.  Will this result in extra practice times?  Fields with lights are very hard to find and in most cases unavailable to soccer.  Many teams that had schedules extend into November in the past did not practice the last few weeks due to lack of daylight.

The re-designed format will strengthen development?
  • This probably is valid.  Dividing each division in half means games between more comparable teams.

Secondly, soccer fields especially in the fall are always in high demand. By limiting the
number of games in the season it would ease the pressure put on these fields and the
administrators who are responsible for them.

A shorter ten game season will reduce demand on fields?
  • Field demand was already reduced last fall when all girls U15-19 WSPL games were moved to spring.
  • Games in September & October will only be reduced because double headers were dropped.  This only affects teams who played teams from Eastern Washington.  In some cases the games will simply be moved to November
  • In many Western Washington areas Turf fields are not available until Nov-Dec because of school sports.  The opening up of these school fields takes some pressure off of the fields.  The move of some Sept-Oct games to November will put pressure on the fields during Nov that was not there last year.
  • The only real reduction in field use will be seen in the month of December.

Finally, Washington Youth Soccer realized that the season’s format presented a burden to
most players and families. As it was currently structured, the fourteen game season
generally coincided with hectic holiday schedules and inclement weather. In addition,
with gas prices and environmental concerns at an all-time high, moving to a ten game
season would hopefully alleviate many of the financial strains put on our members.

Shorter season will save gas.
  • For some Western Washington teams, there will be a gas savings.  The old 14 game home/home schedule would mean they might have to make one or two trips to Eastern Washington.  The new schedule has eliminated trips to Eastern Washington for some teams entirely even though they have up to two Eastern Washington teams in their division.  For Eastern Washington teams, the expension of the seasion through November will result in more gas being used.
  • For Eastern Washington teams, a majority of gas is spent on trips over the pass.  While some Western Washington teams have reduced trips over the pass entirely, Eastern Washington teams are seeing the same number of trips over the pass with fewer games.  A typical schedule would have a team travel across the pass 4 times for 7 games.  The new schedule has them traveling 4 times for 4 games.

“Moving our fall league schedule to ten games aligns us with what we did this past spring
with our high school age girls. This is a decision that takes into consideration our players
and the total numbers of competitive games they play throughout the year. There is
general consensus that youth soccer players should play around thirty games per year.
The decision by Washington Youth Soccer is to move towards this number,” said
Director of Coaching Dave Schumacher. “We understand that for those players that are
playing year round that there needs to be a balance between the number of competitive
games in a year and the amount of rest & regeneration. We have a developmental goal to
provide a quality playing environment for our players and this decision is step in
achieving this goal”

Consensus - youth soccer players should play around 30 games per year.
  • WSPL teams play 10 games.  Add 4 State Cup games.  Leaves 16 additional games per year or about 4 tournaments
  • PDL teams play 20 games.  Add 4 State Cup games.  Leaves 6 additional games per year or about 1 tournament
  • WSYSA Director of Coaching and the VP of Development sit on the PDL Committee.  If WSYSA feels that 14 games is too much, why does the PDL play 20?
